Witness to fatal torrent released from B.C. dam recounts moment 'all hell broke loose'

Metro Vancouver commissioner Jerry Dobrovolny told a news conference Friday afternoon there will be a thorough investigation to determine what caused the gate to open on the Cleveland Dam, saying, "we will leave no stone unturned."
